A reading guide to online care

Visit notes

# Make room for your whole health story.

A short note can help when the call feels rushed. It can be untidy. It only has to help you remember what matters.

## Put these things within reach

- **The reason for the visit.** Describe the concern in your own words. Add what you have already tried.

- **Your health notes.** Bring the history you want to discuss, including allergies, past treatment and any pregnancy plans.

- **Your current list.** Include medicines and supplements. Bring the names rather than relying on memory.

- **Your questions.** Mark the one you most want answered. It can go at the top, even if it is about cost.

## A form has a place, and a limit

The person checking your health needs to be able to ask more questions after reading your form. The Federation of State Medical Boards explains this in guidance checked on 26 September 2026.

Under this guidance, a form with set questions and answers is not enough by itself to base treatment on. A health check that leaves out needed information is not enough either.

State law decides which rules must be followed. States take different approaches as of 26 September 2026.

Ask what happens after you submit a form. Who reads it? Can they ask more? How do you ask your own questions?

## Keep the decision in plain words

“What are we trying to treat?” “What else could we do?” “What would make you decide against this medicine?” Ask them to explain again if you cannot describe their decision in your own words.

## Before the conversation ends

Write down what was decided, what remains open and who handles the next task. Ask for any plan to be sent in a form you can keep.

You must also be able to ask your own questions about the care. The same medical board group calls for this in guidance checked on 26 September 2026.
